District Health Services

The Nurse’s Office is concerned with the overall health and well-being of the student and how his/her health impacts the educational process. It is important for the parent or guardian to keep the nurse informed of any health concerns. All information will be treated with sensitivity and confidentiality. For matters of an urgent nature that need immediate nursing attention please contact the nurse via telephone rather than by email.

Absences are to be reported to the main office of your child’s school. Please state the child's name, grade, teacher, and reason for absence. All other school related health concerns can be answered by the nurse's office.

Health Forms and Requirements

Per New York State laws, ALL medications including those that can be purchased over the counter (such as Advil, Tylenol, etc.) require both a physician’s order and parent authorization. Please be advised that all medications must be delivered to the Health Office in the original pharmaceutical container by an adult with a prescription label bearing the student’s name.  Children/students cannot deliver or carry medication to or from school unless they have physician authorization to do so and in consultation with the school nurse. No medication will be accepted without orders and no orders will be accepted without medication.

Sports Requirements (Grades 7-12)

All Sports Clearances are done through the Arbiter Sports (FamilyID) platform.

  • If you haven't already done so, you need to create an account.
  • When creating an account, please use a parent email address. Parent and student signatures are required in the registration process.
  • If you already have an account with Family ID, registration is opened for each specific season approximately one month prior to the start of tryouts.
  • Physical examinations are valid for an entire 12 months extending through the sport season in which the physical date expires. The notification from Family ID that your physical is expiring should serve as a reminder to make an appointment for your next physical exam. A new document is only needed if the previous physical exam has expired as indicated in Family ID or if you have not yet submitted one to Family ID or the Nurse's Office.

Sports Physicals

  • The school doctor is scheduled to be on campus prior to each sports season to do physical examinations for student athletes that do not have a current physical on file.
  • The parental permission form must be completed in order to be examined by the school doctor.
